In-Kind Donation Log and Request for Thank You Letter Form.  Any board member or staff member can use this on-line form to log the receipt of an in-kind donation (all donations must be board approved).  Whoever accepts the donation, must fill out this form.  The form will be used to generate the appropriate Thank You Letter for tax purposes and for our financial records.
Request for Reimbursement.  Print out the attached form, fill it out, attach the original receipt, and give it to the treasure (or put it in the Treasurer's box).  Keep a copy for yourself.  If you have filled our one of these forms in the current fiscal year (July to June), you do not need to put your address on it again.
